1. The Stallion Service Fee of $_______.00 is due prior to shipment of semen.  Any fees incurred in the process of shipping must be paid to Countryside Farms at the completion of shipping and prior to receiving a Breeder's Certificate.

2. The Mare Owner agrees to abide by the Countryside Farms Procedural Guidelines for breeding with Transported Semen.

3. The Stallion Manager assumes responsibility only to deliver live Semen in a viable condition at the time of delivery to the Mare Owner's farm.  The Stallion Manager agrees to ship a maximum of two shipments of live semen in a viable condition per cycle, for a maximum of three cycles or until the end of the breeding season, whichever occurs first.

4. Countryside Farms Live Foal Guarantee is based upon full and complete adherence to the following conditions (live foal meaning standing and sucking without assistance):

   • All shipping charges MUST be paid in full.

   • The Mare MUST be pregnancy checked and found to be in foal at 30 - 50 days by a certified Veterinarian.  If the Mare is found to be open, a Veterinarian Certificate MUST be mailed to the Stallion Manager within 10 days.

   • In the event that the Mare fails to produce a Live Foal, the Stallion Manager MUST be notified by certified mail or by telephone within 48 hours of the Foal's death.  In addition, the Breeder's Certificate MUST be returned to the Stallion Manager within 10 days of the Foal's death.

   • Countryside Farms agrees to offer and additional transported Semen shipment of a refund of the Service Fees, minus a $_______.00 Booking Fee and any transport costs in the succeeding year only, for any Mare that fails to produce a live Foal.  The Mare owner agrees to pay a second Booking Fee of $______.00 as well as any additional transportation fees incurred for additional Stallion Service.

5. It is further agreed that should the Stallion die or become unfit for service, or if any Mare should die or become unfit to breed, then this contract shall become null and void, and any money paid as the Service Fee shall be refunded to Mare Owner.
6. The Mare Owner shall assume all responsibility for the condition of the Mare and shall bear all risk of loss or damage to the mare whether by death, disease, injury or otherwise and by any cause whatsoever and therefore agrees that the Stallion Manager will not be held responsible for any and all damages associated herewith.  if for any reason the Mare does not settle, Countryside Farms will not be held responsible.
7. A Photostat copy of a Mare Registration Certificate (if registered, both sides thereof, must be returned with this contract as one of the requirements).
8. The Mare Owner understands that Countryside Farms will ship Stallion Semen only between February 1 and July 31.
